Osteoarthritis

Gout

Gout is an inflammatory form of arthritis caused by urate crystals that collect in and around a joint. It often produces sudden, intense pain, swelling, redness, and warmth. The big toe is a common location, but gout may also affect the ankle, knee, or another joint. 

Infection

A bacterial or viral infection can cause joint discomfort. An infection within a joint may lead to rapid swelling, severe pain, warmth, and fever. This combination needs prompt medical attention because delaying treatment can allow damage to develop.

Pain from nearby structures

Discomfort that feels like joint pain may sometimes begin in a tendon, bursa, muscle, or nerve. A spinal problem can also send pain into a shoulder, hip, arm, or leg. A clinical examination helps determine whether the symptoms originate within the joint or another structure.

Common clues worth discussing with your provider include:

  • Pain after a recent injury or increase in physical activity
  • Swelling, redness, warmth, or tenderness
  • Stiffness after waking or sitting for a long period
  • Clicking, locking, grinding, or instability

  • Fever, rash, fatigue, or unexplained weight changes
  • Reduced strength or range of motion

When Inflammatory Arthritis May Be Responsible

Arthritis is a broad term for conditions that affect joints and surrounding tissues. NIAMS describes it as joint inflammation that may cause pain, redness, heat, stiffness, or swelling. Different forms have different causes and treatment plans, so identifying the type matters. 

Inflammatory arthritis occurs when the immune system or another inflammatory process affects joint tissue. Unlike temporary soreness after activity, inflammatory symptoms may continue during rest, involve several joints, and produce prolonged morning stiffness.

Rheumatoid arthritis

Rheumatoid arthritis is an autoimmune disease in which the immune system attacks the lining of the joints. It can cause swelling, stiffness, pain, and reduced function. Symptoms often follow a symmetrical pattern, affecting both hands, wrists, knees, or feet. 

Without proper management, persistent inflammation may contribute to cartilage and bone damage. Diagnosis usually draws on medical history, a physical examination, blood tests, and imaging. A rheumatology referral may be appropriate when symptoms suggest an autoimmune condition.

Psoriatic arthritis

Psoriatic arthritis may affect people with psoriasis, although skin and joint symptoms don't always begin at the same time. Possible signs include swollen fingers or toes, heel pain, stiffness, nail changes, and discomfort in the spine or peripheral joints.

Reactive arthritis

Reactive arthritis can develop after certain infections. It frequently affects the knees or ankles and may also cause discomfort in the lower back or where tendons and ligaments attach to bones. 

Other autoimmune and inflammatory conditions

Lupus and other rheumatic diseases may affect multiple joints along with other areas of the body. Gout also produces inflammation, although its cause differs from autoimmune arthritis.

The following patterns may suggest an inflammatory source:

  1. Morning stiffness lasts much longer than brief stiffness after sleep.
  2. Multiple joints are swollen, warm, or tender.
  3. Symptoms improve somewhat with gentle movement but worsen after long periods of rest.
  4. Fatigue, fever, rash, eye symptoms, or other systemic changes occur.
  5. Pain returns in flares or appears in matching joints on both sides.

These signs don't confirm a diagnosis. A provider must consider the full symptom pattern and determine which tests are appropriate.

What Pain Patterns Can Tell Your Healthcare Provider

The location and behavior of joint symptoms often provide useful diagnostic clues. Keeping notes for several days can help you describe what's happening accurately.

Tell your provider whether the pain began suddenly or gradually. Sudden joint pain can be associated with an acute injury, gout flare, infection, or another rapidly developing problem. Gradual discomfort may be related to osteoarthritis, repetitive strain, or a chronic inflammatory disorder.

The number of affected joints is also useful. Pain in one knee after a fall suggests a different cause from stiffness in both hands and feet. Multiple joint pain without swelling may still deserve evaluation when it persists, limits daily activity, or occurs with fatigue and weakness.

A brief symptom record may include:

  • The date and time each episode begins
  • Which joints hurt and whether both sides are affected
  • Pain severity and the type of sensation
  • The length of morning stiffness
  • Visible swelling, warmth, redness, or bruising
  • Recent illnesses, injuries, travel, or medication changes
  • Activities that improve or worsen the discomfort

When to Seek Medical Care for Joint Pain

Occasional discomfort after unusual activity may improve with rest and a gradual return to movement. Joint symptoms that persist, recur, or interfere with normal function should be discussed with a healthcare provider.

Schedule an evaluation when:

  • Pain, swelling, or stiffness continues for several days
  • Symptoms repeatedly return
  • More than one joint is affected
  • Morning stiffness lasts longer than expected
  • A joint feels unstable, locks, or loses movement
  • Pain interrupts sleep or makes daily tasks difficult
  • Home care provides little or no improvement
  • You have a personal or family history of autoimmune disease

Joint symptoms accompanied by fever, rapid swelling, marked warmth, severe redness, or an inability to move the joint require prompt care. Seek urgent evaluation after a serious injury or when you can't put weight on the affected area. A visibly deformed or dislocated joint is a medical emergency. 

A healthcare provider may review your medical history, medications, activity level, prior injuries, and family history. The physical examination may assess swelling, tenderness, stability, strength, range of motion, and the condition of surrounding tissues.

Blood tests can help investigate infection, inflammation, gout, or autoimmune conditions. Imaging may be ordered when the provider needs to inspect joint spaces, bone alignment, cartilage, or soft tissue. No single test explains every type of joint pain.

Questions about vitamin deficiencies are also common. Nutritional problems can contribute to musculoskeletal symptoms, but joint pain alone can't identify what you may be lacking. Taking high-dose supplements without testing can create risks or interact with medications. Ask a healthcare provider whether laboratory testing or dietary guidance is appropriate.

How Diagnostic Imaging Evaluates Joint Health

Medical imaging is selected according to the symptoms, suspected condition, and joint being examined. The goal is to answer a focused clinical question, not simply to scan every painful area.

X-rays are often the first imaging study used for persistent joint symptoms. They can show fractures, alignment changes, narrowed joint spaces, bone spurs, and some signs of arthritis. They don't provide the same soft-tissue detail as MRI.

MRI creates detailed images of cartilage, ligaments, tendons, muscles, bone marrow, and other soft tissues. A provider may order an MRI to evaluate a suspected tear, subtle injury, inflammation, nerve problem, or joint abnormality that isn't fully visible on an X-ray.

Ultrasound uses high-frequency sound waves to produce images from inside the body. It can assess fluid, soft-tissue structures, tendon movement, and inflammation around certain joints. It may also help guide injections or other procedures when clinically appropriate. 

CT imaging produces cross-sectional views of bones and other structures. It may be useful for complex fractures, detailed bone assessment, or areas that are difficult to evaluate fully with standard X-rays.

A radiologist interprets the images and prepares a report for the referring provider. Your clinician then compares the findings with your symptoms, physical examination, and laboratory results.

Imaging may help:

  1. Detect fractures, cartilage loss, or other structural changes.
  2. Assess the location and extent of an injury.
  3. Identify inflammation or fluid around a joint.
  4. Distinguish between possible causes of persistent symptoms.
  5. Support treatment planning or monitor disease progression.

A normal scan doesn't mean the pain is imaginary or unimportant. Some conditions don't produce clear imaging findings, particularly during an early stage. Your provider may recommend physical therapy, laboratory testing, medication management, rheumatology care, or another evaluation based on the complete clinical picture.

Managing Chronic Pain and Protecting Your Joints

Treatment depends on the diagnosis. Measures that help mild overuse discomfort may not be suitable for an infection, fracture, autoimmune disease, or gout flare.

For temporary soreness without warning signs, a provider may recommend activity modification, gentle movement, and adequate sleep. Heat may help stiffness, while a wrapped cold pack may reduce swelling after a recent minor injury. Avoid placing heat or ice directly against the skin.

Rest and movement often need to be balanced. Complete inactivity can increase stiffness and reduce muscle support around a joint. Once serious injury has been ruled out, controlled exercise or physical therapy may help improve strength, stability, flexibility, and function.

Medication may be part of a treatment plan, but pain relievers and anti-inflammatory drugs aren't safe for everyone. Kidney disease, stomach problems, blood-thinning medication, pregnancy, allergies, and other factors may affect which products are appropriate. Follow your provider’s advice rather than relying on prolonged self-treatment.

Depending on the cause, care may involve:

  • Physical or occupational therapy
  • Changes to repetitive activities or workplace ergonomics
  • Medication for arthritis, gout, or inflammation
  • Bracing or another supportive device
  • Weight management when recommended by a clinician
  • Image-guided injections or procedures
  • Referral to orthopedics, rheumatology, pain management, or another specialty
  • Surgery for selected structural problems that don't respond to other care
